FAQ-012: How will this affect the specificity of LR-5 for diagnosis of HCC?
The revised criteria are slightly less stringent; some specificity reduction is expected. The reduction is likely to be small, however, as prior studies have shown that in high-risk patients, the combination of 10-19 mm size, nonrim APHE, and nonperipheral “washout” provides > 90% specificity for HCC.

FAQ-019: A 10-19 mm observation with nonrim APHE, nonperipheral WO, no enhancing "capsule" and no TG was previously categorized LR-4 using version 2017. Although the observation is unchanged in size and in all other features, it is now categorized LR-5 using v2018. How should I report the change in category despite stability in features?
In the body of the report, describe all the major features and ancillary features (if applicable), indicating stability since the prior study. In the Impression, report that although stable in size and appearance, the observation meets revised criteria for LR-5 (definite HCC) per version 2018.
